Job Description:
Monday-FridayNo weekends or holidays!Remote - must reside in MO,
AR or OKOverview:
The Diabetes Care Nurse will work in
active partnership with the patient, family, provider, and clinical
staff to develop and implement a well-coordinated and innovative
plan of care that ensures appropriate education, goal setting,
control and maintenance of their disease.This position is integral
to Mercy's efforts to provide high-quality patient care and
implement successful care delivery methodologies needed for
alternative payment models and value-based care.Qualifications:
- Education: RN with a BSN or equivalent experience
required.
- Experience: Prior work experience in Case Management, Disease
Management, or Managed Care.
- Certification: CDCES certification preferred.
- Licensure: Current RN licensure in the applicable state of
